C-section overuse in Mexico driven by hospital specializations and obesity

Cesarean section (C-section) procedures have increased dramatically around the world in the recent decades. Overweight and obesity rates, common risk factors for pregnancy outcomes and for C-sections, are also on the rise -; creating a major health issue in low- and middle-income countries. A new study from the University of Illinois Urbana-Champaign investigates how high obesity levels lead to hospital specializations that affect the frequency of C-sections in Mexico.

Of course, obesity is a medical factor for C-sections; however, when we started this project we did not believe it to be the only explanation for the high rates. Our hypothesis was that hospitals with a higher pool of obese patients become specialized in C-sections, and therefore will be more likely to perform the procedure.”

The study correlates patient data from 226 public hospitals across Mexico from 2008–2015 with birth records that contain information on birth-type delivery, birth outcomes, and maternal characteristics.

The main analysis focuses on mothers with low-risk pregnancies to eliminate obvious medical reasons for a C-section. Thus, the sample excludes women under 18 and over 35, those carrying multiple children, births under 37 weeks of gestation, and women with more than 20 prenatal visits.

The researchers found that the individual probability of a woman having a C-section is correlated with high levels of the obesity measure at the hospital level, supporting the hypothesis that doctor specialization leads to increased use of the procedure.

C-section rates in Mexico doubled from 23 percent in the 1990s to 46 percent in 2016, which is well beyond the World Health Organization’s (WHO) recommended rate of 10-15%. At the same time, at least 72 percent of the adult population in the country is overweight or obese.

“Mexico has one of the highest C-section rates in Latin America, but there are huge differences across different sectors. The private sector uses C-sections very intensively, while it’s used a lot less in public hospitals,” said co-author Fernanda Marquez-Padilla, Centro de Estudios Economicos, El Colegio de México.

The research team ruled out other factors driving the increases in C-sections, including maternal characteristics such as education and age. To exclude the possibility of women selecting a hospital based on specialization, the study limited the analysis to municipalities with no more than one facility. Moreover, as these are public hospitals where payment doesn’t depend on procedure, there are no monetary incentives for doctors.

“The study is conducted within a public healthcare setting that is resource constrained and hence the lessons we learn from this context are likely to be informative for other developing countries. The findings are likely to be useful for countries that are following a similar path in terms of increasing C-section rates and obesity trends,” Marquez-Padilla noted.

The hospital data do not include obesity information for individual women, so the researchers constructed a measure that allows them to gauge the overall obesity rate for hospitals. They used patient discharge data to identify obesity-related diseases, such as hypertension, diabetes, and heart disease, to create a hospital-level obesity index.

We built this obesity measure from discharge records using ICD codes, the WHO medical classification of health data. Our paper makes an important methodological contribution as we design a reliable obesity proxy, which can be used by other researchers in different countries, as long as there is hospital discharge data available.”

Patient outcomes

The researchers also investigated the consequences of the increase in C-sections and what it means for maternal and infant health.

“We divided birth outcomes into delivery-related and delivery-unrelated, as we would only expect to see improvements in outcomes that doctors can affect,” Marquez-Padilla said. “We do in fact find positive effects for mother survival, decreases in birth injury and birth trauma, and improvements in Apgar scores, which indicates a baby’s health at birth. Furthermore, we show there’s basically no effect on birth weight, which is unrelated to delivery type, except for very heavy babies, which correlate with obesity and overweight in the mother.”

The study also found better outcomes for vaginal births as C-sections increased. This is likely because the riskiest vaginal births are moved to the C-section group, so the average risk of women delivering vaginally is decreasing, Marquez-Padilla said.

The researchers emphasize that these findings do not indicate C-sections are always preferable.

“C-sections are good when they’re necessary, but they shouldn’t be used when they’re unnecessary. In an ideal world, you will want high-risk women to go to the highly specialized C-section hospitals and low-risk women to deliver at hospitals that specialize in vaginal births,” Herrera-Almanza said. “One policy implication is that patients should be identified and sorted in terms of the procedure they need and be directed to the best hospital for that procedure.”

Reducing maternal and infant mortality are part of the United Nations’ Sustainable Development Goals, and C-sections can help save the lives of mothers and children. However, overuse may lead to negative health consequences and waste of resources. Thus, it is important to understand how to optimize the use of the procedure in developing countries, the researchers concluded.

Canales Furniture; International investment; Another castle; Bruceville-Eddy drive-in theater

Canales Furniture, a family-owned company based in Fort Worth, has signed a lease on the former Dirt Cheap space at 300 N. Valley Mills Drive, according to Waco real estate agent Randy Reid, who brokered the deal.

Fort Worth-based Canales has grown to about 20 locations in Texas and Oklahoma, according to its website. It will occupy 34,995 square feet in the old Dirt Cheap building located next to Planet Fitness. That Waco Commons Shopping Center also is home to a Tractor Supply store.

Reid also is listing the former CVS Pharmacy store at New Road and Valley Mills Drive that just went vacant. Some might believe the space hard to fill considering it was built to accommodate a niche user.

But Reid said he already has several interested parties.

International investment

Kris Collins, the Greater Waco Chamber of Commerce’s senior vice president for economic development, shared an interesting statistic with business leaders Friday, when she presented the latest Greater Waco Economic Index findings.

The GWEI declined in September for the seventh straight month. But Collins softened the blow by reminding that Waco has become a melting pot for industrial development. Seven new companies from five countries have placed plants in Greater Waco since 2020. Announcements by companies from Mexico, Canada, Germany, Spain and Thailand have created $455 million in spending, 1.5 million square feet of new construction, and 578 new jobs.

Castle market update

Chip and Joanna Gaines took their Cottonland Castle off the market, deciding to resume public tours at the landmark Austin Avenue property.

But would-be castle owners still have options. The Parsons Castle at Lake Whitney recently became available, priced at $5.5 million. It features 10 bedrooms, 15 bathrooms, 11,000 square feet, a ballroom, commercial kitchen and bar, and bride and groom suites, according to a listing by Hacker Property Group. The owners reportedly engineered and built the home over 10 years as a private residence that could accommodate nuptials.

The owners “have held weddings there … but never heavily marketed it as event space,” said Eric Hacker with Briggs Foreman Sotheby’s International Realty. Hacker said he has marketed other properties considered castles, “but I’ve never seen anything that overlooks a lake like this.”

The property lies in Bosque County, and has a Clifton address.

Drive-in theater

Pamela Cervantes was down but not out on Monday. Her new drive-in movie theater in Bruceville-Eddy did not attract the crowds she had hoped for over the weekend. In fact, she and family members who bundled up in a pickup truck probably outnumbered the outsiders who attended.

“I don’t understand why the community didn’t come out,” said Cervantes, who wondered if Friday night football games cut into her grand-opening crowds. She thought she had received a positive response from those she told of her plans. She worked overtime spreading the word on social media and even had a couple local TV stations out to enthusiastically tell her story.

Her husband, Julian Cervantes, owns a Dallas-based construction company specializing in building highway bridges for the Texas Department of Transportation. He committed money and manpower to making the 19-acre drive-in theater setting presentable and accessible. Stage, screen, projector, sound system, concession stands and bouncy houses stood ready.

But few came, and reviews were not encouraging. C&C Theater took online heat for its pricing. Several posters compared it unfavorably with the drive-in theater in Gatesville. Pamela Cervantes said in an interview the Gatesville drive-in is a different animal, operating throughout the week and not just on weekends. She charged a little more for admission so she could charge less for food and drink, she said. But the community has spoken, so she has made price adjustments for this Friday and Saturday evenings.

Kids to age 4 and seniors over 65 pay nothing. Four people or fewer pay $15 a carload, more than four pay $20 a carload, she said in an interview. The Cervanteses do not have permits to show first-run movies, so November is “Hallmark” month and December will showcase holiday favorites.

Pamela Cervantes said the movie site would hold about 150 cars under ideal parking conditions. The exact address is 2355 N. Old Bruceville Road, which is about a 4-mile drive from Interstate 35. Cervantes suggested taking interstate Exit 321, hitting Callan Ranch Road, and watching for signs.

Charging at Buc-ee’s

Buc-ee’s and Mercedes-Benz are teaming up to give motorists a charge from their driving experience. The maker of luxury automobiles announced in a press release it has partnered with the exploding Texas-based chain “in launching a network of premium EV charging stations” across North America.

Once found exclusively in Texas, Buc-ee’s now has locations in Alabama, Florida, Georgia, Kentucky, South Carolina and Tennessee. It has broken ground on new stores in Colorado, Missouri and Mississippi.

“Buc-ee’s strategic locations along major travel routes, combined with their commitment to clean and accessible amenities, aligns perfectly with our vision. Together, we’re not only providing exceptional value but also redefining the EV charging experience for drivers nationwide,” Mercedes-Benz North America CEO Andrew Cornelia said in the press release.

The nearest Buc-ee’s location to Waco is in Temple, but another is under construction about 30 miles up Interstate 35 in Hillsboro.

THE OPENING TIP        

• The last time out, NM State handled business with a comeback win over Northern Colorado 76-71.  

• The Aggies are looking to make a statement in the Cancun challenge ahead of its contest against Fresno State. They will be taking on either James Madison or SIU depending on the outcome.

• The Bulldogs are coming off a home win against Morgan State at home 87-68. They are looking to build some momentum against the Aggies Tuesday evening. 

FRESH START            

• Entering the season, NM State is the only program in the country with an entirely new roster as zero players from the 2022-23 campaign will take the floor for the Aggies this season.

• Tasked with building a roster from scratch that is also expected to be competitive immediately, Head Coach Jason Hooten opted to bring in guys with experience as all but one Aggie on this year’s roster has played basketball beyond the high school level. This includes 12 individuals with collegiate experience and one with experience in the NBA Global Academy. Of the 13 players that have collegiate experience, eight have experience at the Division I level.

NEW YEAR, NEW LEADER         

• After spending the last 13 seasons as the bench boss at Sam Houston, Jason Hooten takes over an Aggie program in need of a refresh button.

• Many are confident Hooten has what it takes to return the Aggie program back to glory as he begins his tenure after compiling 261 wins, posting a .607 win percentage and helping the Bearkats to six postseason appearances. With the success, Hooten ultimately departed Huntsville, Texas, as the program’s all-time winningest coach.

• In his final year with Sam Houston, Hooten helped lead the ‘Kats to the program’s first-ever appearance in the NIT Tournament. Once the Bearkats reached the national tournament, they made some noise – defeating Santa Clara in the opening round. During last year’s historic season with Sam Houston, Hooten also led his Bearkats to upset road wins over a pair of Power Five programs in Oklahoma and Utah. The Aggies will be looking to absorb some of that magic on Monday when they take on their blue blood opponent

ROOM TO GROW         

• In their first season as a member of Conference USA, the Aggies were projected to finish eighth in the nine-team league according to a vote of the conference’s nine head coaches.

• This underdog role is new territory for the Aggie program as they become affiliates with CUSA after a run in the Western Athletic Conference that included seven regular season titles and 10 tournament titles during their 18-year stint as a member of the WAC.

• In fact, during the Aggies’ 18 years in the WAC, NM State was selected to finish inside the top three of the coaches poll every year except for their inaugural season in which they were also tabbed to finish eighth. During that stretch, the Aggies were labeled preseason favorites nine times.

TOURNAMENT TIME        

• NM State’s 10 conference tournament titles since 2006 are the second-most in the nation. Only the WCC’s Gonzaga (14) has more. 

BUILDING A FOUNDATION        

• Although, this may be Jason Hooten‘s first year with the program, the former Sam Houston coach has done his best to surround himself with familiar faces as two staff members and starting center Kaosi Ezeagu from the Bearkats also made the trip from Huntsville to Las Cruces during the offseason.

• Last season, Ezeagu served as a vocal leader on Sam Houston’s squad and started all 33 games that he appeared in and paced the Bearkats in blocks with 16 on the season.

• In addition to Ezeagu, Assistant Coach Payne Andrus and Graduate Assistant Terryonte Thomas were also contibutor’s to the success of the Bearkat program last season.

POWER FIVE AGGIES        

• The 2023 NM State roster features three Aggies who have spent time on Power Five rosters as Kaosi Ezeagu (Kansas State), Femi Odukale (Pitt) and Davion Bradford (Wake Forest and Kansas State) each played among the best of the best during their collegiate careers.

• Unfortunately for the Aggies, only Kaosi Ezeagu will be available in the season opener as both Odukale and Bradford are awaiting transfer waivers from the NCAA.

COOK’S LEADING THE WAY

• Versus Northern Colorado, Christian Cook led the team in scoring with 26 points while shooting 5-7 from beyond the arc. He is currently the highest scorer for the Aggies this season – showing the way with 61 total points while averaging 12.2 points a game.

AGGIES LOCKED IN FROM DEEP    

•The Aggies have been connecting from consistently from the three point line. They are currently shooting .374% from behind the arc which is ranked 74th in the country and second in CUSA.    

POSEY DROPPING DIMES

Jaylin Jackson-Posey is leading the Aggies in assist averaging 4.2 a game with 21 total for the season. This has him ranked 60th currently in the country and third in CUSA.

FEMI’S AGGIE DEBUT

Femi Odukale made his long-awaited NM State debut on Saturday, playing 23 minutes off the bench and recording 18 points, eight rebounds and four steals. Showing his prowess around the rim, Odukale attempted 15 free throws on the night, making nine to lead the game in both categories. His 15 attempts became the most by an Aggie since Ivan Aurrecoechea took 15 shots from the charity stripe on Dec, 9th 2018 against New Mexico. 

SCOUTING THE BULLDOGS    

• Fresno State will take on the Aggies Tuesday in what will be its fourth game of the season coming off a 87-68 win against Morgan State at home.

• Last season, the Bulldogs finished the season with a 11-20 overall record and a 6-12 mark against Mountain West Conference opponents.

• The Bulldogs will be led by 6-4 junior guard Xaiver DuSell. He is currently averaging 15.6 points and 4.3 rebounds a game.

• Fresno State is led by Justin Hutson who is entering his sixth season as the Bulldogs head coach. He is (84-72) in his tenure as head coach and this is his first head coaching job after being a Division 1 assistant coach for 10 season prior to his hiring.

Claudia Sheinbaum gets married in small civil ceremony

Presidential frontrunner Claudia Sheinbaum has tied the knot to Jesús María Tarriba Unger, her former university classmate.

The Morena party 2024 candidate shared her happiness on Instagram last week, first with a teaser photo of two wedding bands and the message “I have news for you…”

On Friday, the couple was married in an intimate civil ceremony before family and friends in Mexico City. “We are so happy,” Sheinbaum wrote on social media.

Their romantic journey was unveiled during a 2022 radio interview, in which the then-mayor of Mexico City disclosed that she had been in a six-year relationship with Tarriba and that the two were engaged.

Their shared history, dating back to their days at the National Autonomous University of Mexico (UNAM), found renewed vigor after years of separation. In 2018, during her campaign for Mexico City mayor, Sheinbaum hinted at the relationship but kept her partner’s identity under wraps.

Last year, Sheinbaum recounted their reconnection in 2016: “I was head of the delegation in Tlalpan [borough of Mexico City]. We saw each other in 2016 around Christmas, and we hit it off very well. He returned to Spain, then he came to Mexico again, and then I went to Spain for about five days to see him there, to see where he lived and so on, and that’s when he tells me, ‘I want to go to Mexico with you.’”

Tarriba, who turned 61 on Sept. 1, has a Ph.D. in physics from UNAM and works as an economic risk analyst.

Sheinbaum, who turned 61 on June 24, earned an undergraduate degree in physics followed by two postgraduate degrees in energy engineering from UNAM. She spent for years at the Lawrence Berkeley National Laboratory in California completing work for her doctoral thesis.

Sheinbaum was previously married, from 1987 to 2016, to politician Carlos Ímaz Gispert. Together they have one daughter, Mariana Imaz, 35, who earned a Ph.D. in philosophy at University of California, Santa Cruz and is now the academic coordinator of humanities at UNAM-Boston. Sheinbaum also has a very close relationship with Rodrigo Imaz, Carlos Ímaz Gispert’s son from a former marriage. 

Sheinbaum is seeking to become the first female president of Mexico, with the election set for June 2, 2024. Her main competitor is Xóchitl Gálvez of the Broad Front for Mexico (FAM).Although Sheinbaum is Jewish, nothing on social media indicated the wedding had any Jewish touches. Sheinbaum is generally very quiet about her Judaism.

Engineering international exchange programs offer opportunity to connect to a global community

An interest in travel drove him to study abroad, but Mechanical Engineering student Robert Gillespy also gained technical skills, career focus and new contacts from the experience. Gillespy studied at the Technical University of Dresden in summer 2022 and at CETYS University in Mexico in summer 2023. The programs are offered through the College of Engineering’s International Exchange Program.

In Germany, Gillespy had a paid internship working on experimental aerodynamics: he conducted experimental testing on model wings in a wind tunnel. The experience, he said, “really guided my desire to get into aerodynamics and fluid dynamics. Germany was a big stepping stone for what I wanted to do in my career.”

Gillespy and two other University of Nevada, Reno students took part in the program, which connected them with a global community.

“There were students from around the world there, so it was a good opportunity to meet students from diverse backgrounds,” Gillespy said.

Gillespy’s other study abroad experience was a two-week program at CETYS University in Ensenada in Baja California, Mexico. The international program brought engineering and business students from around the world to study together and network. This time he was the only one from the University to go.

“Mexico was a unique opportunity,” Gillespy said. “Since it wasn’t so engineering-oriented, it gave me good insight into the business operation side of things. Part of the events we did was touring the manufacturing plants, (delving) a lot into the operation sides, which was a different perspective on engineering as a whole.”

Those types of experiences augment an engineering education, according to Julie McMillin, who coordinates the College of Engineering’s International Exchange Programs.

“Besides academic rigor, studying abroad can help you develop the “softer” skills that employers are looking for,” McMillin said. “Skills like independence, teamwork, leadership and adaptability.” 

Gillespy credits McMillin for introducing him to study abroad and allaying some of his concerns about studying abroad, particularly around financing.

“There are a lot of opportunities out there to receive funding,” Gillespy said. “Finances are a big part of (studying abroad), but another big part of it is just living smart while there.” 

Fiscal management wasn’t the only thing Gillespy learned in the field: he realized the importance of making global connections, even if there’s a language barrier.

“Going somewhere where you can’t speak the language is challenging,” Gillespy said. “Figuring out how to adapt to a foreign environment was a big change. Still … once you make those friendships, they are strong.”

New Mexico Supreme Court Weighs GOP Challenge to Congressional Map, Swing District Boundaries

SANTA FE, N.M. (AP) — The Republican Party urged New Mexico’s state Supreme Court on Monday to strike down a congressional map that has divvied up a politically conservative oil-producing region into multiple districts as it reshaped a swing district along the U.S. border with Mexico.

The high court heard oral arguments without ruling Monday on the congressional map from Democratic state lawmakers. The Democrats say a congressional swing district in southern New Mexico remains competitive, even with the ouster of a Republican incumbent in last year’s election.

The high court’s ruling could influence which party represents the state’s 2nd Congressional District, where Democratic U.S. Rep. Gabe Vasquez is seeking a second term.

The district is one of about a dozen in the national spotlight as Republicans campaign to keep their slim U.S. House majority in 2024. Courts ruled recently in Alabama and Florida that Republican-led legislatures had unfairly diluted the voting power of Black residents. Legal challenges to congressional districts are also ongoing in Arkansas, Kentucky, Louisiana, South Carolina, Tennessee, Texas and Utah.

A New Mexico state district judge ruled in October that Democratic state lawmakers substantially diluted the votes of their political opponents, but that the changes fell short of “egregious” gerrymandering.

Appealing that ruling, the Republican Party said its substantial political minority in New Mexico is likely to be shut out of congressional representation for the entire decade before maps are redrawn again. As evidence, the GOP cited the 2022 defeat of incumbent GOP Congresswoman Yvette Herrell to a former city councilman from Las Cruces.

“Herrell was obviously an incumbent who had been on the ballot multiple times with very high name ID and she lost at the end of the day,” Harrison told the justices on Monday. “So a very Republican-favoring year, with an incumbent.”

Justices voiced skepticism, noting that Herrell lost by a thin margin of 0.7% in 2022 and also previously lost an open race for the seat in 2018 before the district was redrawn — indications that the district was competitive and may remain so.

“The actual election results, that’s the one that I’m having trouble with,” Justice Briana Zamora said.

Sara Sanchez, representing Democratic legislative leaders, said evidence in the case doesn’t support allegations of an egregious gerrymander that would entrench one party in power or deprive voters of meaningful participation.

“Every map is going to favor one party over the other in any given district,” she said. “But vote dilution only becomes a constitutional injury when it rises to the level of effectuating that entrenchment, and there just was not evidence of that here.”

Democrats hold every statewide elected office in New Mexico, along with its three congressional seats and two Senate seats.

Foreign direct investment in Mexico’s auto industry surges

Foreign direct investment (FDI) in Mexico’s automotive industry surged in the first three quarters of 2023 to reach a record annual high three months before the end of the year.

Economy Ministry (SE) data shows that US $5.44 billion in FDI was allocated to the manufacture of cars, trucks and other vehicles between January and September, a 67.7% increase compared to the same period of 2022.

The previous annual record for auto sector FDI was $4.29 billion in 2019. By the end of September, that amount had already been exceeded by 26.7%

The increase in FDI in the automotive industry comes as more and more foreign companies that operate in the sector take the decision to establish a presence in Mexico or expand their existing operations here.

Tesla’s announcement in March that it would build a $5-10 billion electric vehicle “gigafactory” in Nuevo León was undoubtedly the most prominent development in Mexico’s auto sector this year, but a number of other companies have also recently decided to invest significant amounts of capital here.

They include BMW, which announced in February that it would invest 800 million euros in San Luis Potosí to produce high-voltage batteries and fully electric “Neue Klasse” vehicles and Volkswagen, which is planning an initial $763.5 million investment to modernize production processes at its plant in Puebla.

Among the new auto sector plants that have opened in Mexico this year is an $80 million one built by Chinese parts manufacturer Yinlun in Nuevo León.

Proximity to the United States, competitive labor costs and benefits associated with the USMCA free trade pact all make Mexico an attractive location for auto companies.

A total of just over $39 billion in FDI has flowed into Mexico’s automotive industry since 2013, according to SE data compiled by the El Economista newspaper.

That investment has helped spur production, which, in turn, has allowed vehicle exports to increase.

Automakers with factories in Mexico exported 301,341 new vehicles in September, the highest monthly figure since June 2019, while 112,261 new cars were sold in Mexico in October, the strongest performance for that month since 2018.

According to the International Trade Administration, a U.S. government agency, Mexico’s automotive industry employs over 1 million people and contributes to 3.6% of the country’s GDP and 18% of manufacturing GDP.

Mexico marks 113 years since the revolution

Today marks the 113th anniversary of the Mexican Revolution, a critical event in Mexico’s history that helped shape the country’s modern political and social structures.

Here are the basics you need to know about the Mexican Revolution.

When was the Mexican Revolution?

The Mexican Revolution was an armed conflict that began in 1910 as a rebellion against General Porfirio Díaz’s prolonged rule. However, it quickly escalated into a civil war with various factions vying for control of the revolution. An estimated 2 million people died in the conflict, which continued until 1920. 

Why is Revolution Day on Nov. 20?

When President Porfirio Díaz was elected for another term in 1910, former candidate and liberal leader Francisco I. Madero launched a plan to overthrow him.

The plan had a main motto: “Effective suffrage, no re-election.” It demanded labor rights and land distribution, which were sought after by social groups against Díaz.

According to the Chronology of the Revolution by the National Institute of Historical Studies on the Revolutions of Mexico (Inehrm), the plan called for an armed struggle on Nov. 20, from 6 p.m. onwards, in which all citizens of the Republic were called to take up arms to overthrow Díaz.  

Nationwide, however, 13 armed struggles had already begun before 6 p.m. on that day, marking Nov. 20 as the start of the Mexican Revolution.

Some of the most important – and familiar – historical figures of the conflict include Pancho Villa, Venustiano Carranza, Emiliano Zapata, Álvaro Obregón, Victoriano Huerta and Francisco I. Madero.

Women also played an important role in the revolution, supporting and even participating in armed combat on all sides. They were known as Las Adelitas.
